Health and Nutrition Assistance for Women, Infants and Children (WIC Program)

Harmony Healthcare Long Island’s WIC program is a supplemental health and nutrition program specifically for expectant mothers and their children (up to the age of 5).  You and your family may be eligible for assistance in providing nutritious foods, learning the basics of good nutrition and breastfeeding guidance and leading to a lifetime of healthy habits.

How WIC can Help You

If you qualify, the WIC program could provide:

  • Nutritional guidance and menu plans specific to your family goals
  • One on one breastfeeding coaches educate and encourage a very personal and gratifying mother and child experience
  • Financial assistance in the purchase of select food and childcare necessities
  • Aiding in the identification and use of community services

Our WIC Locations

Who is Eligible?

  • Women who are pregnant, breastfeeding or postpartum women.
  • Children who are under five years old (including foster children).
  • Families with low to medium income (working families may qualify).
  • Parents or guardians who are the sole provider of children under the age of five who are at nutritional risk and who are at or below 185 percent of the Federal poverty level.
The WIC Program provides health and nutrition assistance.

How to Apply

Applying for the New York State WIC program is simple and easy.

What You Need to Bring to Your Appointment

When you arrive, one of our representatives will verify your documentation and ask a few brief questions to verify your eligibility. Your child must be present at your first WIC program appointment.

Please bring the following documents:

  • A completed New York State Department of Health Medical Referral Form from your health care provider, including proof of pregnancy and/or blood test results.
  • Proof of identity for each person applying for the WIC program.
  • Proof of residence (an actual street address, not a post office box).
  • Proof of income for each member of your household who is working (documentation must be for the last 30 days).
  • Proof of participation in Medicaid, SNAP/Food Stamps or Temporary Assistance to Needy Families (TANF).
