Created by the New York State Department of Health to help children and adults receive assistance regarding concerns about their overall health and wellness. Care Coordination integrates an individual’s physical health, behavioral health and social service needs under the direction of a designated care coordinator.

Your specific Care Coordinator works with you to develop a comprehensive approach for improving your health and well-being. Medical care, employment, housing, education and nourishment may be some areas to address.

FAQs about Patient Care Coordination

1)      Who is Eligible?

At Harmony Healthcare Long Island, Care Coordination Services are available to both children and adults. To be eligible, an individual must meet a few basic requirements:

  • Live in Nassau County
  • Eligible to receive Medicaid
  • Diagnosed with at least two chronic health conditions, including:
  • Diabetes
  • Asthma
  • Heart disease
  • High blood pressure
  • Substance abuse
  • Obesity
  • HIV/AIDS
  • A serious emotional disturbance
  • Complex trauma

2)      What Does a Care Coordinator Help With?

If you enroll in Patient Care Coordination, you will receive a designated Care Coordinator to oversee and coordinates access to health services, such as medical, behavioral health and social services.

Nine Types of Assistance You Can Receive

  1. Applying for benefits
  2. Accessing resources, social support, and activities in your area
  3. Understanding your illness
  4. Communicating with doctors, nurses, and other providers
  5. Coordinating transportation to your appointments
  6. Understanding and acquiring your medications
  7. Finding employment or returning to school
  8. Providing connections to other financial assistance programs
  9. Finding appropriate and affordable housing

3)      How will my Health Services be Delivered?

This program is a convenient health care option for individuals who have two or more chronic health conditions. Your Care Coordinator will be easily accessible in many ways, including:

  • Face to face at our offices or coming to your home
  • Communicating by phone
  • Exchanging emails or text messages

4)      Will my Patient Information Be Kept Private?

Harmony Healthcare Long Island takes all measures possible to ensure the protection of the privacy of all patients regardless of immigration status.

If you enroll in our Care Coordination program, none of your personal information will be shared unless you agree in writing.  You may also consent in writing that designated family members or friends may be kept informed of your participation in the program.

All your personal health information will be fully protected under HIPAA laws.

5)      Will I have to Change my Doctors?

No. Enrolling in Patient Care Coordination should not interrupt your current health care services, such as the doctors you are using or your Medicaid Managed Care Plan.
